Form of Request for Reconsideration. In order to be considered, a Request for Reconsideration must be filed with the Finance Director in writing, with copies provided to the CPO and Contract Specialist, and include: (a) Name, address, and telephone number of the person protesting or their authorized representative; (b) A copy of the original Protest, including supporting documents; (c) A copy of the written decision of the CPO; and (d) Include all pertinent facts and law on which the Protester is relying.
