Common use of Formal Level I Clause in Contracts

Formal Level I. 1. If, as a result of the informal discussion with the immediate supervisor, the alleged grievance still exists, the employee or Union representative may invoke the formal grievance procedure on a form signed by the grievant and/or Union representative, which form shall be available from the local Union representative. A copy of the completed and signed grievance form shall be delivered to the employee's non-Union immediate supervisor (or designee) within three (3) working days after oral discussion. 2. The written grievance shall include (as a minimum): a. Identification of the grievant(s) b. The specific facts upon which the grievance is based c. Identification of the specific portion of the Agreement allegedly violated d. The specific relief requested e. The date on which the alleged grievance occurred
