Group Dental Plan. 20.3.1 The premiums for the dental plan will be paid seventy percent (70%) by the Library and thirty percent (30%) by the employees whose contributions shall be made by payroll deduction. Basic dental service (Plan A) paying for eighty percent (80%) of the approved schedule of fees. 20.3.2 Restorative dental service (Plan B) paying for fifty percent (50%) of the approved schedule of fees. 20.3.3 Effective January 1, 2014, Orthodontics (Plan C) – Employer pays fifty percent (50%) of approved schedule of fees. The orthodontic lifetime maximum per child shall be two thousand dollars ($2,000.00). Coverage shall only apply to dependent children.
