HEALTH OF CHILD. In permitting my child to swim with Occoquan Swimming (hereafter referred to “OCCS”), I affirm that my child has no known medical problems and/or conditions that prevent(s) his/her full participation in OCCS activities and programs, and I accept full responsibility for my child's participation in the program. I understand that the OCCS Medical Emergency Form must be completed before my child may be allowed to participate in OCCS activities and programs and that my child's participation must be in accordance with the rules and regulations for swimmer conduct. I agree to immediately notify OCCS in the event of any change in the health of my child, and I understand that I am responsible for deciding whether or not it is appropriate for my child to participate in the program. OCCS reserves the right to deny participation to any child if that child appears to suffer from a contagious disease that could potentially threaten the health of other swimmers, or if participation, at the discretion of the coach, is believed to be detrimental to the child's health. OCCS does not discriminate on the basis of any disability. In signing below, I consent and authorize the administration of emergency medical care on my child in the event that I am not available or cannot be reached.

  • Leave for Birth of Child On the occasion of the birth of his child, a male employee shall be granted special leave with pay up to a maximum of one (1) day during the confinement of the mother. This leave may be divided into two (2) periods and granted on separate days.

  • HEALTH CARE PLANS ‌ Notwithstanding the references to the Pacific Blue Cross Plans in this article, the parties agree that Employers, who are not currently providing benefits under the Pacific Blue Cross Plans may continue to provide the benefits through another carrier providing that the overall level of benefits is comparable to the level of benefits under the Pacific Blue Cross Plans.

  • Abuse and Neglect of Children and Vulnerable Adults: Abuse Registry Party agrees not to employ any individual, to use any volunteer or other service provider, or to otherwise provide reimbursement to any individual who in the performance of services connected with this agreement provides care, custody, treatment, transportation, or supervision to children or to vulnerable adults if there has been a substantiation of abuse or neglect or exploitation involving that individual. Party is responsible for confirming as to each individual having such contact with children or vulnerable adults the non-existence of a substantiated allegation of abuse, neglect or exploitation by verifying that fact though (a) as to vulnerable adults, the Adult Abuse Registry maintained by the Department of Disabilities, Aging and Independent Living and (b) as to children, the Central Child Protection Registry (unless the Party holds a valid child care license or registration from the Division of Child Development, Department for Children and Families). See 33 V.S.A. §4919(a)(3) and 33 V.S.A. §6911(c)(3).

  • Health Care Operations Health Care Operations shall have the meaning set out in its definition at 45 C.F.R. § 164.501, as such provision is currently drafted and as it is subsequently updated, amended or revised.

  • Hospitals of Ontario Voluntary Life Insurance Plan The Hospital also agrees to make the Hospitals of Ontario Voluntary Life Insurance Plan (HOOVLIP) available to the nurses subject to the provisions of HOOVLIP at no cost to the Hospital.
