Individual HSA. 1. Effective January 1, 2014, the City shall contribute 55.8% of the deductible ($1116) on behalf of the member to member’s private HSA. The employee may elect to contribute, by payroll deduction on a pre-tax basis, up to $884 per year for the individual medical coverage. The employee may elect not to pay his share of the contribution toward the deductible. Once the City’s contribution is exhausted, the employee shall be responsible for the remainder of the deductible before the City assumes any and all additional healthcare costs incurred consistent with the health plan offered in Section 21. This includes but is not limited to office visits, emergency room fees, prescription deductibles, etc. Once the deductible is met, the City offers 100% co-insurance.
