Common use of Life Insurance Plan Clause in Contracts

Life Insurance Plan. An employee must sign an application form to be eligible for this Plan and coverage will commence on the first of the month following completion of the probationary period. Should application not be made within three (3) months of this initial commencement date, the insurance company will require satisfactory evidence of insurability from the employee and his eligible dependents (i.e., spouse and unmarried, unemployed children between the ages of 14 days and twenty-five years).
