Life Insurance Plan. ‌ Benefits are based on the insurance amount in force at the time of the insured person’s death or dismemberment. The claimant must submit to the Insurer the claim and any supporting documents required by the Insurer within 10 years following the date of the insured person’s death or the accident causing the dismemberment.
