Each graph summarizes trends for the previous 12 months – including the most recent quarter. Quarterly reports should be no more than 4 pages – preferably 2 pages double-sided. Use reports for monthly DHMT CCM implementation update, clinical care meeting and district and provincial quarterly performance review. Distribute reports to facility-based supervisors. Servicer will make good faith efforts to notify Merchant promptly following suspension. Servicer is not liable to Merchant for any loss, either direct or indirect, which Merchant may attribute to any suspension of funds disbursement.

  • Project Monitor The Project shall be monitored by the Sponsor through the Director of the Xxxxxxx-Xxxxxxxxx County Human Services Department (“Project Monitor”). In the event of noncompliance with this Grant Contract by Grantee, the Project Monitor shall report said noncompliance to the Lancaster County Board for further action which may include termination of the Grant Contract.

  • Invoicing for Charges Against the Judicial Council’s Master Account A. The Contractor shall establish a Master Account for the Judicial Council’s charges provided for under the exhibits of this Agreement.

  • Quarterly and Annual Reconciliation 10.6.1 The Parties acknowledge that all payments made against Monthly Bills and Supplementary Bills shall be subject to quarterly reconciliation within 30 days of the end of the quarter at the beginning of the following quarter of each Contract Year and annual reconciliation at the end of each Contract Year within 30 days to take into account the Energy Accounts, Tariff adjustment payments, Tariff Rebate, Late Payment Surcharge, or any other reasonable circumstance provided under this Agreement.
