Mandatory Medical Certification. Any employee requesting leave because of a serious health condition of the employee or family member must furnish the Agency with appropriate medical certification. The certification must be signed by the appropriate health care provider on the form provided by the Human Resources Department. Employees should submit the medical certification form along with their leave request, and the Agency may deny the taking of leave until the required certification is provided. In cases of unforeseen leave, medical certification must be submitted to the Agency as soon as possible. The Agency may, in its discretion, require any employee seeking leave because of a serious health condition to obtain a second opinion at Agency expense and time. When an employee is on leave, subsequent re-certifications of a medical condition must be submitted upon the Agency's request, as reasonably necessary.
