Mark-Off. When an employee is unable to report to work, the employee shall notify the employee’s immediate supervisor or other designated person within two (2) hours (unless extenuating circumstances prohibit) prior to the time the employee is scheduled to report to work on each day of absence, unless other arrangements are made with the employee's supervisor. Upon return to work an employee shall complete an application for sick leave form to justify the use of sick leave. The Employer may, when an employee utilizes sick leave for medical appointments or where an absence is for three (3) consecutive days or more, require the employee to furnish a certificate from a physician, dentist, or other medical practitioner.
