Medical Leave for Extended Illness. An employee is entitled to a medical leave of absence for an extended illness. ·The leave is available for up to one calendar year from the date the leave began. ·To be eligible for the leave, the employee must apply for the leave, in writing, by completing the appropriate Family and Medical Leave Request form, available from the Human Resources Office. ·The application for leave must be completed within the first 5 days of absence, whenever possible.
