Extended Medical Leave. A teacher who is unable to teach because of personal illness or disability and who has exhausted all available sick leave shall be granted leave for up to one (1) year. Any request for this leave must state the probable date of return and be accompanied by a verifying physician's statement. If the leave was granted as a result of a work related injury the employee may request and shall be granted a one (1) year extension of this leave. Before returning, such teacher must submit a physician's release. To be eligible for an extended medical leave of absence, an employee must have been actively employed on paid status with APS a minimum of one contract year immediately prior to the leave start date. All intermittent medical leaves will be addressed according to FMLA regulations.

Extended Medical Leave. A Staff Member may apply for extended medical leave for health reasons for the remainder of a current school year. The request for such leave must be accompanied by a statement from a physician indicating that such leave is necessary for the physical or emotional well-being of the Staff Member. Such leave may be granted upon receipt of said statement, upon the discretion of the Board and compliance with FMLA, USERRA, the ADA and the ADAAA of 2008.
Extended Medical Leave. An employee unable to work because of illness or accident who has exhausted paid sick leave, vacation, PTO, Family Medical Leave, may apply for a personal leave of absence for medical purposes. Applications of such leave shall be made in accordance with County policies. Existence and extent of illness or disability must be verified by a written statement from a health care provider when requested by the EMPLOYER. An employee returning from an unpaid leave of absence for medical purposes shall be placed in the employee's department and classification. The returning employee shall be accorded the treatment due the employee's seniority if the department and/or classification were eliminated during the absence. Employees will return at the same salary (which includes any General Adjustment made during the absence) in the existing salary schedule, will retain promotion rights, and will earn vacation/PTO schedule seniority under this paragraph.
Extended Medical Leave. Should the Minister suffer an illness, injury, or disabling condition that continues after all accrued sick, vacation, and study leave has been exhausted, the Congregation will place the Minister on “Extended Medical Leave.” Extended Medical Leave will not extend beyond the earlier of the commencement of long-term disability benefit or 90 days following the exhaustion of all sick, vacation, and study leave.
Extended Medical Leave. A medical leave of absence without pay may be granted to an employee covered by this Agreement because of non-occupational illness or injury in any of the following events: (A) following exhaustion of any and all sick pay benefits to which the employee may be entitled pursuant to this Agreement; or (B) immediately upon the occurrence of such illness or injury in the event the employee has no sick pay benefits accumulated at that time. A. Such medical leave will be granted upon receipt by the Human Resources Department of the employee’s physician’s certification as to the necessity of such leave and such leaves shall be subject to all of the applicable conditions of Article XIV, section 14.2 and 14.3 of this Agreement.
Extended Medical Leave. An employee who has exhausted any eligible Family and Medical Leave Act (FMLA) job protected leave shall be granted a leave of absence without pay up to one (1) year because of personal illness or disability. The Board shall base its decision as to the illness or disability and the length of leave therefore on the recommendation of the employee’s physician and/or a physician selected by the Board if the Board so desires. At its discretion the Board may renew the leave for up to two years. The Board shall continue to pay their current District paid benefits for the duration of said leave, but not beyond twelve (12) months in all events. Thereafter, but prior to the expiration of the Board-paid insurance coverage, the employee may arrange with the Human Resources office to continue premium payments at his/her expense. It is understood, however, that the Board shall not be responsible and does not insure that the employee can secure such continued coverage with the insurance carrier on the expiration of the first twelve (12) months of leave.
Extended Medical Leave. ‌ A. An extension of unpaid medical leave beyond the limits of state or federal FML may be granted at the discretion of the Authority. B. Such approval shall not be unreasonably denied. C. In the event a discretionary medical leave is granted, and the employee has been unable to work for a total of six months, the employee shall be non- disciplinarily terminated from the Authority. i. Such termination shall not be considered disciplinary in any way. ii. If within one year after such termination the employee becomes capable of performing the required job duties of that previously held position, the employee may return to that position if it is vacant. iii. If the position the employee previously held is not vacant or no longer exists, the employee shall be entitled to placement into a vacant position, or in the next available position if no such vacancy exists, in the same classification within the department or division and for which the employee is qualified. iv. In the event that any employee who has been terminated pursuant to this Article regains a work capacity and returns to work, that employee shall not lose the benefit of any prior years of service immediately preceding termination, for purposes of seniority, vocational accrual rate, and restoration of sick leave credits. D. The Authority shall pay up to three monthly premiums for insurance provided by this contract the employee had when the state or federal family medical leave commenced while the employee is on such discretionary extended medical leave.

Extended Medical Leave. Should the minister’s illness, injury, or disabling condition continue after all accrued sick and vacation leave has been exhausted, the congregation shall place the minister on “Extended Medical Leave.” Extended Medical Leave shall not extend beyond the earlier of either the commencement of long-term disability benefits or 90 days following the exhaustion of all sick and vacation leave. Vacation leave does not accrue during this period. During Extended Medical Leave, the congregation shall continue to pay all employee insurance premiums (health, dental, life, retirement, and long term disability) as otherwise provided by this Agreement, and no less than 75 percent of the minister’s salary and housing allowance. If the minister recovers and is able to return to work full-time before the earlier of the 90-day period of Extended Medical Leave or the commencement of long term disability benefits, the congregation shall retroactively pay the minister at 100 percent of salary and housing. Extended Medical Leave. The Superintendent shall consider requests for extended medical leave that are in excess of the Family Medical Leave Act requirements from district employees that have completed three consecutive years of employment with the district. The district may re- quest verification from the employee’s physician. The length of time shall be limited to one year and shall be without pay after the employee has used all sick leave benefits. Individuals hired to fill the vacancy created by the medical leave will be hired only for the period of the medical leave. Upon return the district may require documentation that the employee has been released from medical care.
Extended Medical Leave. Any employee whose personal illness extends beyond the period compensable under this Article shall be granted a leave of absence without pay for reasons of health up to one (1) year, subject to extension by the Employer. The leave request shall be in writing and supported by a doctor’s statement.
