Network Pharmacies. For Prescription Drugs purchased at a retail or mail order Network Pharmacy, You are responsible for paying the lower of: • The applicable Cost-Sharing; or • The Network Pharmacy’s Usual and Customary Charge (which includes a dispensing fee and sales tax) for the Prescription Drug. (Your Cost-Sharing will never exceed the Usual and Customary Charge of the Prescription Drug.) In the event that Our Network Pharmacies are unable to provide the covered Prescription Drug, and cannot order the Prescription Drug within a reasonable time, You may, with Our prior written approval, go to a Non-Network Pharmacy that is able to provide the Prescription Drug. Network Pharmacies. Unless otherwise provided in Exhibit C-1, et seq., as applicable, or agreed to United in writing for each Covered Prescription Service provided by a Network Pharmacy, Administrator shall charge United or the Member the same amount that Administrator reimburses the applicable Network Pharmacy for the provisions of the Covered Prescription Service (i.e., the amount of the Actual Prescription Drug Reimbursement shall equal the amount of the Pharmacy Drug Compensation). United acknowledges that in the states where United maintains a traditional pricing structure with Administrator, the compensation Administrator pays Network Pharmacies pursuant to Network Pharmacy Agreements may not be the same amount of Prescription Drug Compensation paid to Administrator by Client. Unless otherwise stated in Exhibit C-1, et seq., or unless otherwise agreed upon by the Parties, Administrator shall charge United for the provision of Covered Prescription Services the lesser of (i) the Usual and Customary Charge; or (ii) the Prescription Drug Compensation.
Network Pharmacies. All sub-contracted pharmacies must be licensed by the State(s) in which they operate and all pharmacists performing work at the vendor sub-contracted pharmacies must be state-licensed. A list of all pharmacies under sub-contract with the Contractor will be provided to the VA pharmacy contacts listed in Exhibit 2. Updates to the list of subcontracted pharmacies will be provided to the VA by the Contractor upon request.

  • Pharmacy Services The Contractor agrees to comply with the requirements regarding covered pharmacy and over-the- counter (OTC) benefits. The Contractor will comply with the EOHHS Pharmacy Home Program and the Generics First Initiative, including the maintenance of the drug formulary in accordance with the direction of the EOHHS Pharmacy Committee.

  • Hospice Services Services are available for a Member whose Attending Physician has determined the Member's illness will result in a remaining life span of six months or less.

  • Anesthesia Services This plan covers general and local anesthesia services received from an anesthesiologist when the surgical procedure is a covered healthcare service. This plan covers office visits or office consultations with an anesthesiologist when provided prior to a scheduled covered surgical procedure.
