Common use of NEWBORN COVERAGE Clause in Contracts

NEWBORN COVERAGE. (a) If born from a Covered Pregnancy: i. Provisional coverage: If born from a covered pregnancy, each newborn will automatically be covered for complications of birth, and any injury or illness for the first ninety (90) days after birth up to a maximum of twenty-five thousand dollars ($25,000) with no deductible or coinsur- ance. ii. Permanent coverage: For permanent coverage of a child born from a covered pregnancy, a “Notification of Birth” consisting of the newborn’s full name, gender and date of birth must be submitted within ninety (90) days of birth. Effective as of the date of birth, coverage with appli- cable deductible and coinsurance will then be up to the policy limits. Policy limits for complications of birth relating to a newborn are limited to the maximum benefits described in provision 8 (a) i.
