Vision Coverage. A fully employee paid vision benefit will be available beginning January 1, 2021 subject to agreement by the subcommittee of the Joint Labor Management Insurance Committee to the benefit set determined through the state’s Request for Proposal (RFP) process.
Vision Coverage a. The vision plan benefits available are based upon a Plan 2 program.
Vision Coverage. The City will make the current Vision Coverage available at the employee's cost. Effective January 1, 2016, the City will split the cost of the Vision Premiums 50-50 with members of the bargaining units who opt for this coverage.
Vision Coverage. Vision coverage as provided for in the standard district plan.
Vision Coverage. Under the life of this agreement, an optional and fully employee-paid vision benefit will be available pursuant to contract parameters with the State’s vision vendor.
Vision Coverage. Effective January 1, 2003, the City shall provide a Vision Care Program for employees covered by this Agreement. The annual maximums for this benefit are as follows: Exam 12-Months Lenses 12-Months Frames 24-Months Contact Lenses** 12-Months **Note: Benefits for Contact Lenses are in lieu of benefits for lenses and frames. The maximum amount the City shall be required to pay for the Vision Care Program shall be the applicable Vision Services Plan ($25 Plan B) rate (i.e., employee only, employee plus spouse, employee plus one (1) child; employee plus family). If during the term of this Agreement the premiums for such Vision Care Program are increased, the amount the City contributes shall increase no more than five percent (5%) above the previous calendar year’s contribution amount towards the payment of the monthly premium. Recognizing that the “vision exam” portion of the Vision Care Program duplicates existing benefit provided under Medical Coverage Section 24.1 (Medical Coverage) and 24.2 (Maximum Medical Premium Payments), the Union and the City agree to revisit the Vision Care Program during the term of this Agreement with the expectation that it may be possible to identify a plan that provides a “Hardware Only” benefit (Lenses every 12-months; Frames every 24-months). Such “Hardware Only” plan shall be at a lower City and employee cost than the present Vision Services Plan ($25 Plan B).
Vision Coverage. To the extent a Participant elects, any or all of the Benefit Dollars credited to his Account shall be used to pay the Participant's share of the cost of coverage under the Employer sponsored vision insurance plan.
Vision Coverage. The Board shall provide for a vision care program that provides for eye examinations, lenses and frames. The vision care benefit plan will include one (1) comprehensive vision examination every twelve (12) months, and lenses and frames every twelve (12) months. For each teacher covered by this agreement, the Board shall pay all but
$1. 00 per year of the premium cost for either a single or family plan. Additional costs shall be shared equally between the board and the individual teacher.
Vision Coverage. Effective upon execution of the contract and open enrollment period the County agrees to provide Vision Coverage to all permanent employees with an employee contribution of 15% of the monthly premium equivalent for the employee and applicable dependents. The contribution amount will be paid to the County by the employee via payroll deduction, which deduction is hereby authorized by this agreement.
Vision Coverage. 17.5.1 The Board shall pay 100 percent of the monthly premium for individual or family coverage for full-time unit members.
17.5.2 Coverage shall be maintained at the agreed upon level through the term of this agreement.
17.5.3 After June 30, 2004, the benefits may be reduced, terminated, or an employee contribution required if the monthly premium exceeds $16.00 for individual or family coverage.
