Common use of NEWBORN COVERAGE Clause in Contracts

NEWBORN COVERAGE. (a) If born from a Covered Preg- xxxxx: i. Provisional coverage: If born from a covered preg- xxxxx, each newborn will automatically be covered for complications of birth, and any injury or illness for the first ninety (90) days after birth up to a maximum of twenty-five thousand dollars ($25,000) with no deductible or coinsurance. ii. Permanent coverage: For permanent coverage of a child born from a covered pregnancy, a “Notification of Birth” consisting of the newborn’s full name, gender and date of birth must be submitted within ninety (90) days of birth. Effec- tive as of the date of birth, coverage with applicable deductible and coinsurance will then be up to the policy limits. Policy limits for compli- cations of birth relating to a newborn are limited to the maximum benefits described in provision 8 (a) i.
