Common use of Notification of Sickness Clause in Contracts

Notification of Sickness. The CONFIRE Director, or designee, must be notified within at least two (2) hours prior to the start of the employee’s shift of a sickness on each day of absence, unless under a doctor’s off work order for a specified period of time. In such instance where the employee is under a doctor’s off work order, the employee must only notify the CONFIRE Director, or designee, at least two (2) hours prior to the start of the first day of absence and inform him/her of the length of the absence. The employee will then forward the doctor’s off work order to the CONFIRE Director, or designee, within five calendar days from the first day. Failure to make such notification may result in denial of sick leave with pay. It is the responsibility of the employee to keep the appointing authority informed as to continued absence beyond the first day for reasons due to sickness or occupational disability. Failure to make such notification shall result in denial of sick leave with pay. If the employee receives a doctor’s off-work order and provides notice of same to the appointing authority, the employee is not required to contact the department daily. If the employee does not have an off-work order or has not notified the appointing authority that one has been issued, the employee shall be required to contact the department daily in accordance with the timeframe above.
