Notification of Sickness. 1. To receive compensation while absent on sick leave an employee must notify his/her immediate supervisor in the manner provided for in departmental rules and regulations. 2. When a sick leave absence is for more than three (3) consecutive working days, an employee’s illness must be verified in a written statement from an attending physician or a personal affidavit stating the fact of the medical necessity for the absence. Employees shall furnish any other proof of sickness reasonably required by the department head or the City Manager.
