Notification to HHSC. (A) The Contractor must notify HHSC within the timeframes set forth in section 8.2.5(c).
(B) The Contractor must require that its subcontractors and contractors take the necessary steps to assure that the Contractor can comply with all of the following Incident notice requirements:
(1) Incident Notice
(a) Initial Notice: Within twenty-four (24) hours of discovery, or in a timeframe otherwise approved by HHSC in writing, the Contractor must preliminarily report on the occurrence of an Incident to the HHSC Privacy and Security Officers via email at: xxxxxxx@XXXX.xxxxx.xx.
Notification to HHSC. A. The Contractor must notify HHSC within the timeframes set forth in Section C below.
B. The Contractor must require that its Sub-Contractors and Contractors take the necessary steps to assure that the Contractor can comply with all of the following Incident notice requirements.

Notification to HHSC. (a) Provider will cooperate fully with HHSC in investigating, mitigating to the extent practicable and issuing notifications directed by HHSC, for any unauthorized disclosure or suspected disclosure of HHSC Confidential Information to the extent and in the manner determined by HHSC.
(b) Provider’s obligation begins at discovery of unauthorized disclosure or suspected disclosure and continues as long as related activity continues, until all effects of the incident are mitigated to HHSC’s satisfaction (the “incident response period”).
(c) Provider will require that its employees, owners, managing partners, or contractors or subcontractors (as applicable), comply with all of the following breach notice requirements.
Notification to HHSC. The MCO must notify HHSC within the timeframes set forth in Section (3) below unless HHSC has agreed in writing to an alternate timeframe for notification.
Notification to HHSC. The Dental Contractor must notify HHSC within the timeframes set forth in subsection (c) below unless HHSC has agreed in writing to an alternate timeframe for notification.
Notification to HHSC. (a) The MCO must notify HHSC within the timeframes set forth in Section (c) below unless HHSC has agreed in writing to an alternate timeframe for notification.
(b) The MCO must require that its Subcontractors and Providers take the necessary steps to assure that the MCO can comply with all of the following Incident notice requirements.
(c) Incident Notice:
1. Initial Notice. Within 24 hours of discovery of an Incident that the MCO’s risk analysis has determined has more than a low probability of compromise, the MCO must preliminarily report on the occurrence of an Incident to the HHSC Privacy Officer via email at: xxxxxxx@XXXX.xxxxx.xx.xx using the Potential Privacy/Security Incident Form which is available on the HHSC website. This initial notice must, at a minimum, contain (1) all information reasonably available to MCO about the Incident, (2) confirmation that the MCO has met any applicable federal Breach notification requirements and (3) a single point of contact for the MCOs for HHSC communications both during and outside of business hours during the Incident Response Period.
Notification to HHSC a) The Provider must notify HHSC within the timeframes set forth in Section (c) below.
b) The Provider must require that its Subcontractors and contractors take the necessary steps to assure that the Provider can comply with all of the following Incident notice requirements.

(i) all information reasonably available to Provider about the Incident;
(ii) confirmation that the Provider has met any applicable federal Breach notification requirements; and
(iii) the identity of Xxxxxxxx’s single point of contact for communication with HHSC, both during and outside of business hours during the Incident Response Period.
Notification to HHSC i. Integral Care must notify HHSC within the timeframes set forth in Subsection

2. The DSHS must require that its subcontractors and contractors take the necessary steps to assure that the DSHS can comply with all of the following Incident notice requirements.
Notification to HHSC. 8.7.1. UTHealth must notify HHSC within the timeframes set forth in Subsection 8.7.3 below.
