Common use of Orthodontic Insurance Provided Clause in Contracts

Orthodontic Insurance Provided. During the term of this MOU agreement, employees as defined in Section 1.3, the City agrees to pay the premium for the employee and up to Family coverage if desired. Selection of the carrier is at the discretion of the City. The City provided orthodontic insurance plan provides for a maximum lifetime orthodontic payout of $1,500 for each employee and for each employee’s eligible dependents. For part-time employees, please refer to Article 6.4 regarding Pro-Rata Benefits.
