Health and Hospitalization Insurance. Single Coverage: The District shall contribute a sum not to exceed $8180 per year toward the premium for individual coverage for each full-time employee employed by the District who qualifies for and is enrolled in single cov- erage in the School District’s group health and hospitalization insurance plan. Any additional cost of the premium shall be borne by the employee and paid by payroll deduction.

Family Coverage: The School District shall contribute a sum not to exceed $586.00 per month toward the premium for family coverage for each full-time employee employed by the School District who qualifies for and is enrolled in family coverage in the School District’s group health and hospitalization insurance plan. Health and Hospitalization Insurance. The selection of the insurance carrier and policy shall be made by the school district. Effective 9/1/2024, the School District shall contribute a sum not to exceed $847.00 per month, and effective 9/1/2025 will not exceed $890.00 per month toward the premium for single or family coverage for all full-time employees (25 hours per week) employed by the School District who qualify for and are enrolled in the School District group health and hospitalization plan. Health and hospitalization insurance benefits paid to a married couple of employees in the district shall be as follows, subject to the requirements, restrictions and limitations imposed by the District’s health and hospitalization insurance provider. Such employees who are part-time shall be entitled to the District contributions specified below on a pro rata basis determined according to the amount of their full-time equivalent position. The options are as follows: One Family Health and Hospitalization Coverage for Both Employees The married employees may elect, if eligible, to both be covered by one family group health insurance plan. The District shall contribute a sum not to exceed District’s monthly contribution reference above toward the premium for family coverage for one member of a married couple of full-time employees employed by the School District who qualify for and are enrolled in the School District group health and hospitalization plan. In addition, the District shall contribute a sum not to exceed the lesser of one hundred percent (100%) of the cost of the premium for single coverage or the current District contribution for the other member’s bargaining unit of a married couple of full-time employees employed by the School District who qualify for and are enrolled in the School District group health and hospitalization plan. The cost of the single premium shall be the single premium associated with the same health insurance plan under which the married employees maintain dependent coverage. Health and Hospitalization Insurance. Eligibility. All employees working an average of 30 or more hours per week in accordance with the Affordable Care Act are eligible for benefits in this Article.

Family Coverage: The District shall contribute a sum not to exceed $14,212 per year toward the premium for family coverage for each full-time employee employed by the District who qualifies for and is enrolled in family cov- erage in the District’s group health and hospitalization insurance plan. Health and Hospitalization Insurance. Effective January 1, 2016, the School District will pay a pro-rata portion of the premium for single coverage in the plan selected by the teacher:
a. Maximum District Contribution per Month: Up to $417.82 for any plan.
b. The teacher will pay the difference between the School District’s contribution and the premium for the selected plan.
Health and Hospitalization Insurance. Subd. 1. Effective July 1, 2019, the School District shall make an annual contribution toward the cost of the premium for medical/hospitalization insurance on behalf of each teacher employed by the School District who qualifies for and is enrolled in one of the group medical/hospitalization plans offered. The amount of this contribution shall depend on the plan the teacher selects from among the group plans available through the School District. See Appendix K, page 48, for plan options and corresponding District contributions for each plan.
Subd. 2. In no event shall a teacher receive cash in lieu of all or any part of the insurance premium as outlined in Subd. 1.
Subd. 3. In no event shall the insurance premium as outlined in Subd. 1 be applied to any insurance coverage other than the school district approved health and medical policy.
Subd. 4. Any health insurance premium not covered by the School District's contribution will be deducted through payroll from the teacher's current salary using the Flexible Benefit Plan, unless the teacher elects not to use said plan and notifies the School District.
