Outcome Monitoring. ‌ Outcome monitoring for all programming will be reported to the ACCMP Supervisor at identified reporting intervals and as a regular commitment to and practice of continuous quality improvement. for inclusion in the year-end report. Data will be gathered from existing sources that are dependent on the agency such as TRAILS for human services and Infinite Campus for schools. Data will be shared with any partner agency or community member requesting to view information, once all identifying information has been removed that would compromise confidentiality.

Outcome Monitoring. Outcome monitoring for programming that is supported by KCCIOG either financially or with CMP staff will be reported to the KCCIOG Coordinator at least yearly for analysis and will be 6 of 10 Updated April 2023 presented at an IOG meeting and included in the year-end report. Data will be gathered from existing collections sources that are dependent upon the agency, such as TRAILS for Human Services.   • System Monitoring to ensure safe and continuous operation, the Customer must monitor key services and resource use as recommended by Deswik, and provide Deswik with details of monitoring and any relevant alerts as needed. Services to be monitors include, without limitation, disk space, CPU usage, memory usage, database connectivity, and network utilization.
