Collaborative Management Processes. Pursuant to section 24-1.9-102(2)(e), C.R.S., ROUTT COUNTY IOG has established a collaborative management process addressing risk sharing, resource pooling, performance expectations, outcome monitoring, and staff training which can be found in the By-laws/Procedure Guide attached hereto as a labeled Appendix A. By signing this MOU, the Parties agree to follow and review these by-laws annually to ensure all statutory and rule mandates are met.

Collaborative Management Processes.
Section 1. Risk Sharing Legal and financial risk is fully assumed by the partner agency supplying the service. Those partners who received CMP funding from the ACCMP to provide services, must show in their request for funding that have the legal and financial means prior to funds being disbursed. At least 2 times per fiscal year, ACCMP will receive a full expenditure report compiled by the ACCMP Supervisor and the fiscal agent.
Collaborative Management Processes. The collaborative management processes shall address risk-sharing, resource-pooling, performance expectations, outcome-monitoring, and staff training in order to do the following:
A. Reduce duplication and eliminate fragmentation of services provided to recipients;
B. Increase the quality, appropriateness, and effectiveness of services delivered to recipients, to achieve better outcomes; and
C. Encourage cost sharing among service providers. Jefferson County is working with the Xxxxx X. Xxxxx'x Family to Family model to promote the welfare of children through sustainable partnerships that provide integrated, quality services that are individualized, strength based, family centered and culturally competent. The Division is working with other agencies to increase the array of services available to meet the unique needs of children and families through shared resources. Joint treatment planning through ISSTs is reducing duplication and fragmentation of services and encouraging sharing of risks and costs among agencies. Cross-systems training has been implemented and/or supported through Jefferson County’s “Improving Child Welfare Outcomes through Systems of Care” federal grant and/or partner agencies to increase knowledge that will enhance collaboration and best practice. The Division has a Training Coordinator. Staff attendance at the cross-system trainings from multiple agencies have and will assist in the quality, appropriateness and effectiveness of services to recipients and will assist in improving outcomes. The Training Coordinator also offers a training on the Child Welfare system to other agencies, churches, and groups to educate the community on its practice and need for community assistance. The Division has also been working with family members who have successfully completed their involvement in the Child Welfare System so that these families can participate effectively in decision making forums and train, mentor and advocate for families currently in the Child Welfare system. These family members are helping design and implement various ways of doing business in child welfare in Jefferson County, including being involved in Team Decision Making Meetings, Options and various committees and boards both locally and on the state level. The Division also has a Volunteer Coordinator who is working with the community on providing services to the child welfare population. The collaborative management process shall address risk-sharing, resource-pooling, performance expectations, outcome-monitoring, and staff training in order to do the following:
A. Reduce duplication and eliminate fragmentation of services provided to Recipients;
B. Increase the quality, appropriateness, and effectiveness of services delivered to Recipients, to achieve better outcomes; and
C. Encourage cost sharing among service providers.

Pursuant to section 24-1.9-102(2)(e), C.R.S., Montrose County IOG has established a collaborative management process addressing risk sharing, resource pooling, performance expectations, outcome monitoring, and staff training. This management process shall be utilized by the ISSTs and CMP Prevention Programs when providing services to children and families serviced by the parties to this MOU, designed to reduce duplication and fragmentation of services, increase the quality, appropriateness, and effectiveness of services delivered to families, and encourage cost sharing among services providers. All of which can be found in the By-laws/Procedure Guide attached as Appendix A. a. The partner agencies will refer youth early on to the Youth Engaging in Supportive Services (YESS) and Children and Families Engaging in Supportive Services (FACESS) for purposes of collaborative assessment and service planning. The Team will work to avoid duplication of services, establish a realistic individualized support plan and assure provider follow through. Communication between agencies, providers, and family members and collaborative integration will be priorities of the Team.
b. Organization representatives on the Student Threat Assessment Team (STAT) and the Suicide Prevention Program Team (SPP) will effectively communicate current and past invovlement with youth and families.
