Outcomes and Performance Measures. Grantee shall describe the goals and objectives of the program (include definitive measurement or benchmarks for program stability and effectiveness in determining the overall impact of the program). How many total persons did this organization serve during the past year and how many total people does your organization expect to serve in the coming year? How many TANF eligible clients do you project you will serve and what percentage of success is expected? Grantee shall measure the impact of day-to-day work. The Grantee will be able to make more informed collaborative decisions to align the work of the Grantee to affect more positive performance outcomes and change for the purposes served through this Contract. As to each Performance Measure, the Grantee shall collect data monthly to include in the Performance Report. Eligibility, Federal, and Monitoring and Evaluation guidelines (including reporting requirements) have been defined in the initial application material and are incorporated herein by reference.
