Patient Request to Associate Clause Samples
Patient Request to Associate. If a client requests an accounting of disclosures directly from Associate, Associate will make its records of disclosures available to the client and will be responsible to prepare and deliver the records of disclosure to the client. Associate will not provide an accounting of its disclosure directly to the Individual.
Patient Request to Associate. If a Patient requests and accounting of disclosures directly from Associate, Associate will forward the request to Valleywise Health within five (5) business days of Ass and will make its records of disclosures available to Valleywise Health as otherwise provided in this Section. Valleywise Health will be responsible to prepare and for delivery of the records of disclosure to the Patient. Associate will not provide an accounting of its disclosure directly to the Individual.

If a Patient requests an accounting of disclosures directly from Associate, Associate will forward the request to Maricopa County ▇▇▇▇ ▇▇▇▇▇ Part A Program within five business days of Associate’s receipt of the request, and will make its records of disclosures available to Maricopa County ▇▇▇▇ ▇▇▇▇▇ Part A Program as otherwise provided in this Section. Maricopa County ▇▇▇▇ ▇▇▇▇▇ Part A Program will be responsible to prepare and delivery the records of disclosure to the Patient. Associate will not provide an accounting of its disclosure directly to the individual. .
