Practice Monitor. While on probation, Respondent shall work with a practice monitor 23 preapproved by the Board's Executive Director who will provide professional guidance and input with 24 Respondent with respect to his practice in performing evaluations, informed consent, recordkeeping and billing 25 practices. The terms of the appointment of a practice monitor shall be determined by the Board at the time of 26 licensure.

Practice Monitor. While on probation, Respondent shall work with a 15 Practice Monitor pre-approved by the Board’s Executive Director or designee and who will provide professional guidance and input to Respondent with respect to ethics, clinical documentation, recordkeeping, responding to subpoenas, managing a high caseload, good judgment, competency and any other topic deemed pertinent by the Practice Monitor. While on probation, Respondent shall establish and maintain a relationship with a 22 practice monitor who is a masters or higher level behavioral health professional licensed by the 23 Arizona Board of Behavioral Health Examiners at the independent level. The practice monitor 24 shall provide training and assistance to Respondent regarding setting up appropriate forms and 25 formats for Respondent’s clinical records, implementing current behavioral health standards of 1 practice related to behavioral health assessment and treatment planning, providing treatment 2 consistent with the documented treatment plan, and documenting the treatment provided in 3 accordance with current behavioral health standards. Respondent and the practice monitor shall 4 review the clinical documentation produced for each and every active client Respondent sees at 5 least once per month. The practice monitor shall ensure that Respondent complies with the 6 audit plan approved by the Board Chair or designee. Respondent shall meet with the practice 7 monitor a minimum of once a month for the first 12 months of probation. Early release may be 8 requested after 12 months at the recommendation of the practice monitor
Practice Monitor. Within 30 days of the effective date of this Order, Respondent shall 6 submit a professional compliance auditor who is an optometrist in good standing and licensed by the 7 Board,. for approval by the Board or its designee. The focus of the audit shall be in the areas of 8 assessment, diagnosis, treatment, and follow up. The auditor shall also monitor Respondent’s practice for 9 compliance with the Arizona Optometric Practice Act. 10 Respondent shall submit a random sample of at least ten (10) patient medical records for audit to 11 be completed monthly. The auditor shall provide a quarterly report to the Board with audit findings and a 12 remediation plan addressing any deficiencies found during the audit.
