Clinical Documentation. County shall provide Imagine Medical Group, PC, information outlining County’s performance standards and required performance thresholds including, but not limited to,
1. Expectations regarding the pace at which patients should be scheduled
2. Expectations related to timeliness of documentation (e.g., notes due within 24 hours patient’s appointment, etc.) Supplied Personnel will complete and submit clinical documentation to County in a timely manner. County Organization agrees to monitor data validation and immediately inform Supplied Personnel and Imagine Medical Group, PC, of any deficiencies. Imagine Medical Group, PC, is not responsible for requiring Therapists to correct any deficiency reported more than 90 days after the date of service. If County fails to provide performance data on a regular and timely basis, Supplied Personnel cannot be expected to improve deficiencies should they occur.
Clinical Documentation. All clinical professional services will be documented in the medical record, and all documentation will comply with applicable payer regulations. At a minimum, the medical record should establish medical necessity and only reflect treatment for services actually rendered.
Clinical Documentation. Contractors providing Medi-Cal services shall provide and maintain clinical documentation and practice standards that comply with regulatory requirements and with ACBHD Clinical Documentation Standards as specified in the ACBHD QA Manual. Updates and/or clarifications to clinical documentation standards may also occur via ACBHD QA website and publications, including memos, FAQs, and training materials.
Clinical Documentation. Ensure continued quality improvement for accurate and complete coding, resulting in increase in revenue and careful compliance with effective documentation. Provide pre-billing audit of DRG change, Physician/ CDS/ Coder Training Program or CDI comprehensive program implementation that includes a DRG Integrity tool and interim CDI roles performed by AH specialist team.
Clinical Documentation. The State has defined clinical documentation standards for all Medi-Cal providers. These will be provided to CONTRACTOR as a part of the Outpatient Managed Care Clinic Provider Manual. The COUNTY will provide training on these requirements as needed. All charts are subject to state and county audit and Quality Improvement reviews. Lack of compliance with standards can result in disallowances. The State Department of Mental Health requires all Medi-Cal clinics to provide service data to the state CSI system. The COUNTY will work with CONTRACOR to set up data collection and submission processes. These data can also be used to document services provided under the, terms of this Agreement. Reports and access to data for management information purposes will be made available to CONTRACTOR. Data collection is both a requirement as well as a potential tool which can assist CONTRACTOR with program management, completion of Cost Report, etc.
Clinical Documentation. The following changes in project specifications have been made in respect to study documentation: · ICON will write the Austrian Protocol Amendment required for EC and regulatory submissions. · ICON will organise for the translation of medication instructions through a third party vendor. · ICON will organise for the translation of the study protocol into Spanish through a third party vendor. · ICON will organise for the translation of regulatory documentation through a third party vendor.
Clinical Documentation. When requested, Supplier will provide Services to an Eligible Recipient to implement and/or maintain a clinical documentation improvement program. The goal of the clinical documentation improvement program is to make sure that patient medical records contain complete and accurate clinical information to allow for the accurate coding and billing of patients. Supplier will provide these Services to the Eligible Recipient based on a time and materials basis based on the current published rates and reflecting a discount that is no less favorable than that received by other similarly situated customers receiving similar Services and takes into account the existing and future volume of business between the parties.
