Privacy Notice Statement. Prior to collecting PII or other information from Consumers for the purpose of fulfilling a CDO/CAC Authorized Function, CDO and/or CAC must provide Consumers with a privacy notice statement. The privacy notice statement must be in writing and must be provided on, or simultaneously with, any electronic and/or paper form the CDO and/or CAC will use to gather and/or request PII or other information from Consumers. The privacy notice statement must also be prominently and conspicuously displayed on the CDO’s public facing Web site, if applicable, if the CDO and/or CAC will gather or request PII or other Consumer information through that Web site.

Privacy Notice Statement. Prior to collecting PII, the Enrollment Assister must provide a notice that is prominently and conspicuously displayed on a public facing website, if applicable, or on the electronic and/or paper form the Enrollment Assister will use to gather and/or request PII. The statement must be written in plain language and provided in a manner that is accessible and timely to people living with disabilities and with limited English proficiency. 3.4.1 The statement must contain, at a minimum, the following information: a. the legal authority to collect PII; b. the purpose of the information collection; c. to whom XXX might be disclosed, and for what purposes; d. the authorized uses and disclosures of any collected information; e. whether the request to collect PII is voluntary or mandatory under the applicable law; and f. the effects of non-disclosure if an individual chooses not to provide the requested information. 3.4.2 Enrollment Assister shall maintain its Privacy Notice Statement content by reviewing and revising it as necessary on an annual basis at a minimum, and before or as soon as possible after any change to its privacy policies and procedures. 3.4.3 If the Enrollment Assister operates a website, it shall ensure that descriptions of its privacy and security practices, and information on how to file complaints with PHIEA and the Enrollment Assister, are publicly available through its website.

Privacy Notice Statement. In order to process your application to be a Civilian Volunteer in support of authorized Cadet activities, we require certain personal information from you. This Statement explains the purposes and use of your personal information. Only information needed for the purposes of determining your suitability to be a Civilian Volunteer will be requested. The collection and use of personal information for this volunteer application is authorized by section 46 of the National Defence Act and article 3.11 of Chapter 2 of the Queen’s Regulations and Orders for the Canadian Cadet Organizations and is required for your participation. This collection of personal information is not required by law, but is permitted by law should you voluntarily choose to participate in the Cadet Program. However, a refusal to allow us to collect, use and disclose your personal information will result in your not being permitted to participate in the Cadet Program. We will use your personal information to determine your suitability to participate as a volunteer in support of cadet activities. This information may be shared with other DND / CF organizations as well as with the Navy League of Canada, The Army Cadet League of Canada and the Air Cadet League of Canada to ensure the screening process is applied uniformly throughout the Canadian Cadet Organizations. The legislative authority for sharing the information can be found in section 46 of the National Defence Act, article 3.11 of Chapter 2 of the Queen’s Regulations and Orders for the Canadian Cadet Organizations and subsection 8(2) of the Privacy Act.
