Professional Errors and Omissions Liability Insurance a. Coverage shall be in an amount of not less than one million dollars ($1,000,000) per occurrence/aggregate.

Professional Errors and Omissions Liability Insurance. A policy of Professional Errors and Omissions Liability Insurance appropriate to Contractor’s profession in an amount of not less than $1,000,000.00 per claim or occurrence/ $2,000,000.00 general aggregate. If coverage is written on a claims-made form then: (1) the “retro date” must be shown, and must be before the beginning of contract work; (2) insurance must be maintained and evidence of insurance must be provided for at least five years after completion of the contract work; and (3) if coverage if cancelled or non-renewed, and not replaced with another claims-made policy form with a “retro date” prior to the contract effective date, then Contractor must purchase “extended reporting” coverage for a minimum of five years after completion of contract work.
Professional Errors and Omissions Liability Insurance. The limit of liability shall be at least $1,000,000. If the policy is written on a “claims made” basis, Contractor must maintain required coverage for a period of three years after the expiration of this agreement. Contractor may satisfy this requirement by renewal of existing coverage or purchase of either prior acts or tail coverage applicable to said three-year period. Contractor shall submit to Agency certificates of insurance and endorsements for the policies listed above. All endorsements shall be signed by a person authorized by that insurer to bind coverage on its behalf. Agency has the right to require Contractor’s insurer to provide complete, certified copies of all required insurance policies. Contractor shall not cancel, assign, or change any policy of insurance required by this agreement or engage in any act or omission that will cause its insurer to cancel any insurance policy required by this agreement except after providing 30 days prior written notice to Agency. If an insurance policy required by this agreement is unilaterally cancelled or changed by the insurer, the Contractor shall immediately provide written notice to the Agency and obtain substitute insurance meeting the requirements of this agreement. Nothing in this subsection relieves Contractor of its obligation to maintain all insurance required by this Contract at all times during the term of the agreement.

Coverage shall extend to the Locum Tenens Provider(s) through the Professional Errors and Omissions Liability Insurance secured by the Contractor. Coverage shall be in an amount of not less than one million dollars ($1,000,000) per occurrence and three million dollars (3,000,000) aggregate.

The professional errors and omissions liability insurance policy shall embody and include an extended claims period of no less than three (3) years following expiration of this Agreement and any amendments and extensions thereof. If coverage is written on a claims-made form then: (1) the “retro date” must be shown, and must be before the beginning of contract work; (2) insurance must be maintained and evidence of insurance must be provided for at least five (5) years after completion of the contract work; and (3) if coverage if cancelled or non-renewed, and not replaced with another claims-made policy form with a “retro date” prior to the contract effective date, then Consultant must purchase “extended reporting” coverage for a minimum of five years after completion of contract work.

A professional (errors and omissions) liability insurance policy covering the Consultant, each Other Consultant and each other person performing any part of the Design Services, with each policy having policy limits of not less than $2,000,000 per occurrence and $3,000,000 in aggregate and a deductible of not more than $50,000, protecting against all claims for loss or damage arising out of any wrongful act or error or omission in the performance of the Design Services, which insurance shall be maintained in place for at least [time].
