Certificates of Insurance and Endorsements. Consultant shall provide certificates of insurance with original endorsements to City as evidence of the insurance coverage required herein. Certificates of such insurance shall be filed with the City on or before commencement of performance of this Agreement. Current certification of insurance shall be kept on file with the City at all times during the term of this Agreement.
Certificates of Insurance and Endorsements. 1. One or more Certificates of Insurance (“Certificates”) shall be completed by Developer’s insurer as evidence that policies providing the required coverages, conditions and minimum limits are in full force and effect, which Certificates shall be provided to the City for review and approval prior to commencement of any work under this Agreement.
2. Each Certificate required herein, except Workers’ compensation coverage, shall name the City and its officers and employees as additional insureds.
3. Completed Certificates shall be sent to: Risk Manager City of Commerce City Human Resources Department 0000 X. 00xx Xxx. Commerce City, CO 80022 City Engineer City of Commerce City Public Works Department 0000 Xxxxxxxx Xxxxxx Commerce City, CO 80022
4. The City shall review the Certificates and endorsements as soon as practical, typically within ten (10) working days of receipt. The Certificates shall identify this Agreement and shall provide that the coverages afforded under the policies shall not be canceled, terminated or materially changed until at least thirty (30) days prior written notice has been given to the City.
5. In the event the period of coverage for any insurance required herein expires prior to the conclusion of Developer’s obligations hereunder, Developer shall, not less than thirty (30) days prior to the expiration of any such insurance coverage, provide the City with a new certificate of insurance and endorsements evidencing new or continuing coverage in accordance with the requirements of this Agreement.
Certificates of Insurance and Endorsements. Consultant shall furnish SANDAG with Certificates of Insurance and any required endorsements affecting coverage required by this section. The endorsements are to be signed by a person authorized by that insurer to bind coverage on its behalf. Endorsements must specifically state that they modify the policy language. All certificates and endorsements are to be received and approved by SANDAG before work commences. Consultant shall ensure that the representations made on the Certificate of Insurance and Endorsements are true and correct. Complete policies of insurance shall be provided to SANDAG upon request and no later than five (5) days after such request. Failure to provide SANDAG a copy of the insurance policies required in this contract shall be deemed a material breach of contract, which shall be deemed at the option of SANDAG to constitute a cause for immediate termination hereof. The coverage forms and related endorsements required herein required by Consultant shall conform to the Insurance Services Office (ISO) 2013 Edition (or the latest revision available).
Certificates of Insurance and Endorsements. As of the Effective Date, Tenant shall deliver to District a certificate of insurance evidencing the existence of the policies required hereunder and copies of endorsements stating that such policies shall:
1. not be canceled or altered without thirty (30) days prior written notice to District;
2. insure performance of the indemnity set forth in Section 17 (Hold Harmless/Indemnification), above;
3. state the coverage is primary and any coverage by District is in excess thereto;
4. contain a cross liability endorsement; and
5. include a separate endorsement naming District as an additional insured. At least thirty (30) days prior to the expiration of each certificate, and every subsequent certificate, Tenant shall deliver to District a new certificate of insurance consistent with all of the terms and conditions required in connection with the original certificate of insurance as described above.

Contractor will obtain insurance policies issued by an insurance company authorized to do business in the State of Oregon with a minimum financial rating of an AM Best rating of A- or higher. Contractor’s liability insurance, except for professional liability insurance, will be arranged on an “occurrence” basis. The Contractor will be financially responsible for all pertinent deductibles, self-insured retentions and/or self- insurance. Upon request by University, Contractor must provide to University a Certificate of Insurance from the insuring company evidencing insurance coverage required by this Contract. The “Description of Operations” must include (using the following exact language) the “State Board of Higher Education acting by and through the University of Oregon, their officers and employees” as additional insured. The certificate will provide that the insurance company will give a 30-days’ written notice to University if the insurance is cancelled or materially changed. Upon request by University, Contractor will provide to University an endorsement from the insuring company, naming (using the following exact language) the “State Board of Higher Education acting by and through the University of Oregon, their officers and employees” as additional insured. If requested, complete copies of insurance policies will be provided to University.

Certificates of Insurance and Endorsements. 12.6.1 Prior to the Commencement Date, Contractor shall deliver to District a certificate of insurance evidencing the existence of the policies required hereunder stating policies will be occurrence based and stating those insured, extent of insurance, locations and operations to which insurance applies, expiration date, to whom cancellation and reduction notice will be sent, and length of notice period and copies of endorsements stating that such policies shall:
12.6.1.1 not be canceled or altered without thirty (30) days prior written notice to District;
12.6.1.2 insure performance of the indemnity obligations set forth in this Agreement;
12.6.1.3 state the coverage is primary and any coverage by District is in excess thereto;
12.6.1.4 contain a cross liability endorsement; and,
12.6.1.5 include a separate endorsement naming District and its officers, employees and agents as additional insured.
12.6.2 At least thirty (30) days prior to the expiration of each certificate, and every subsequent certificate, Contractor shall deliver to District a new certificate of insurance consistent with all of the terms and conditions required in connection with the original certificate of insurance as described above. Contractor or any of its insurance providers, upon written request of the District or its liability coverage provider, shall provide a copy of the policy and all endorsements in effect at the time of the request, or at the time of a loss as specified by the request. The copy of the policy and endorsements shall be provided within 15 days of the written request.
Certificates of Insurance and Endorsements. As evidence of the insurance coverages required herein, SP shall furnish acceptable insurance certificates and endorsements to CLIENT prior to commencement of any work hereunder. For any work performed hereunder, the insuring company’s certificates and endorsement shall name CLIENT as “additional insured”. Notice of cancellation or change. There shall be no cancellation, material change, reduction of limits or intent not to renew the insurance coverage(s) without thirty (30) days’ written notice from SP or its insurer(s) to CLIENT.
