Program Benefits and Services. PremierMED agrees to provide the following “Program Benefits and Services” in exchange for the Membership Fee: o Same business day appointments o Direct physician visits ▪ At many doctor’s offices the patient meets with a nurse practitioner or a physician assistant, and not a doctor. PremierMED PRIME members meet with a doctor during all office visits. o Convenient dedicated membership appointment slots ▪ Certain high demand appointment times are blocked off for PremierMED PRIME members.

PremierMED PRIME agrees to provide the following Amenities/Benefits (Insurance Non-Covered Services) in exchange for the Membership Fee: o Direct communication to us or our assistant during business hours. • When you call our office during office hours, we strive to answer all phone calls, but for any voicemails, calls will be returned promptly. If you deem your problem “urgent” we will make every effort to speak to you at the time of your call.
