Common use of Program Coverage Clause in Contracts

Program Coverage. Provider agrees to submit pretreatment estimates, if required in the Covered Benefits Schedule. Provider acknowledges that possession of an Avesis identification card or Sponsor identification card does not guarantee that an individual is still a Member on the date services are rendered, or that a Member is necessarily covered for any or all services or materials to be provided.
