Reinstatement Requirements. If a person is the subject of a positive substance test, his/her worker eligibility status immediately becomes “not current” and remains so until all of the following conditions are satisfied: (a) The SAP notifies theTPA in writing that the person has met all conditions stipulated by the SAP that are required, in the professional opinion of the SAP, for the person to safely return to or commence his/her duties as an employee. (b) The person completes return to work testing in accordance with the terms of this Policy to the satisfaction of the TPA. (c) The person agrees in writing to continue any treatment, counseling or rehabilitation as prescribed by the SAP. (d) The person agrees in writing to be subject to Probationary Status/Follow-up testing for two years at a 50% random selection rate.
Reinstatement Requirements. An individual with an Inactive Suspended Status will be reclassified to the Active Pool when all of the following conditions are satisfied: 1. Evidence is submitted to CDS that the individual has completed or is actively participating in an approved drug/alcohol assessment, treatment, and/or counseling program. 2. The employee agrees upon return to submit a urine, screen to the Administrator to determine the existence of foreign substances and/or alcohol within the system and such screening tests is negative. 3. The individual submits a signed statement to CDS that requires him to continue the prescribed treatment
Reinstatement Requirements. The reinstatement of a teacher on parental or adoptive leave shall be contingent upon the teacher being certified and qualified in those activities and disciplines for any existing vacancy, and provided further, the rejection of an assignment when offered shall void the leave protection and shall automatically be treated as a voluntary resignation.

The Substance Abuse Professional (SAP) notifies the TPA in writing that the individual has completed or is actively participating in a Program/Company approved drug and/or alcohol assessment, treatment, and/or counseling program and that the individual is released to return to duty. 2. The employee agrees prior to returning to work to submit a drug and alcohol test through the TPA to determine the existence of foreign substances and alcohol within the system and such screening tests is negative. The cost of this test shall be borne by the employee. 3. The individual agrees in writing to the TPA to continue the prescribed treatment, counseling or rehabilitation as required by the SAP. If the individual does not complete the required treatment, he/she will be subject to disciplinary action, up to and including immediate termination. 4. The individual agrees to be subject to Probationary Status/Follow-up Testing.
