REPORTING PROVISIONS. A. By January 29, 2016, the University will provide OCR a copy of its written guidance, which was developed pursuant to section III.A.1 and 2 of the agreement, for OCR’s review and approval. B. Within 60 days of receiving OCR’s approval of the written guidance, the University will provide OCR with documentation of the following, pursuant to section III.B.1 and 2 of the agreement: 1. Documentation regarding the steps taken by the University to provide the written guidance to agencies, organizations or entities currently receiving significant assistance from the University and to relevant University staff involved in such activities; and 2. The process that the University has put into place for providing the written guidance to any program, agency or organization to which the University plans to provide significant assistance, including incorporating the written guidance into future contracts and memoranda of understanding with relevant programs, agencies or organizations associated with the University. C. By January 15, 2016, pursuant to section III.C.1 and 2 of the agreement, the University will notify OCR with a written report regarding its decision whether to continue to provide significant assistance to the SEYH program. If the University decides that it will not continue to provide significant assistance, the report will include documentation demonstrating the actions taken by the University to immediately terminate significant assistance. D. By February 15, 2016, if the University determines that it will continue to provide significant assistance to the SEYH program under section III.C.3 of the agreement, the University will provide OCR with a written report detailing its review of the practices and promotional and registration materials of the SEYH program, the response from the SEYH program to its request for enrollment and registration records, the results of its review, and the steps it will take to ensure that the SEYH program does not discriminate on the basis of sex, pursuant to section III.C.4 of this agreement. E. Within 30 days of receiving OCR’s approval of the University’s review of the SEYH materials under section III.C.4 of this agreement, the University will provide OCR with a written report detailing the steps taken by the SEYH program to correct its practices, including the documentation provided by SEYH to the University demonstrating the modified practices.

By January 29July 15, 20162014, in accordance with section III, A, 4 of the agreement, the University Seminary will provide OCR a copy of its written guidance, which was draft revised policies and procedures developed pursuant to section III.A.1 and 2 III, A of the agreement, for OCR’s review and approval. B. Within 60 45 days of receiving OCR’s approval of the written guidancerevised policies and procedures submitted pursuant to section III, A of the agreement, the University Seminary will provide OCR with documentation to substantiate that it has adopted and published the OCR-approved policies and procedures. The report will include a copy of the following, published policies and procedures and a link to the Seminary’s website where the policies and procedures are referenced. C. Within 15 days of completing the staff training pursuant to section III.B.1 III, B, 2, the Seminary will provide a report to OCR that reflects the specific actions taken to comply with the training requirements of that section. The report will include the identity and qualifications of the trainer utilized by the Seminary, a copy of the materials used for the training, the date or dates of the training, and a roster of the individuals who attended the training, including the staff persons’ position or title with the Seminary. D. Within 45 days after completing the training required in Section III, B, 2 of the agreement: 1. Documentation regarding the steps taken by the University to provide the written guidance to agencies, organizations or entities currently receiving significant assistance from the University and to relevant University staff involved in such activities; and 2. The process that the University has put into place for providing the written guidance to any program, agency or organization to which the University plans to provide significant assistance, including incorporating the written guidance into future contracts and memoranda of understanding with relevant programs, agencies or organizations associated with the University. C. The report will include a written report detailing its copy of the letter sent to the applicant under section III, C, 1, and a summary of the steps that the Seminary has taken to ensure a “blind” review in the event that the applicant chooses to reapply. E. In the event that the applicant chooses to reapply, within 60 days following the completion of the Seminary’s review of the practices and promotional and registration materials of the SEYH programapplicant’s request for admission, the response from the SEYH program to its request for enrollment and registration recordsSeminary will provide OCR with documentation that that includes, the results of its reviewthat application process, and documentation that his application was reviewed utilizing the steps admission criteria used for all applicants and that it was completed at no cost to the applicant. F. If the applicant provides written notice to the Seminary that he does not intend to accept the Seminary’s offer to reapply, within 60 days the Seminary will take provide documentation to ensure OCR that the SEYH program does not discriminate on Seminary has refunded to the basis of sexapplicant all application, pursuant to section III.C.4 of this agreement. E. By January 2912/15/15, 2016, the University OHSU will provide OCR with a copy draft of its written guidancethe revised policies, which procedures and practices pursuant to section III.A, above, for OCR’s review and approval. OCR will review the policies, procedures and practices and notify OSHU whether any additional revisions are required to comply with Section 504 or Title II. If revisions are required, OHSU will make the revisions and re-submit the draft to OCR. OCR and OHSU will follow the same procedure until OCR approves the policies, procedures, and practices. B. Within 20 school days after receiving approval from OCR regarding the revised policies, procedures and practices, OHSU will provide OCR with a report confirming that OHSU has adopted the approved policies, procedures and practices, including documentation that provides the date and method used to adopt them. C. By 12/15/15, OHSU will provide OCR with a draft of the notice that was developed pursuant to section III.A.1 and 2 of the agreementIII.B, above, for OCR’s review and approval. B. D. The report will include the name and position title of each person that received the specified notice. E. By 12/15/15, OHSU will provide OCR with a description of the tracking system described in Section III.C, above. F. By August 1, 2016 and August 1, 2017, OHSU will provide OCR with a report documenting the provision of all academic adjustments for students with disabilities in the university’s medical school during the preceding school year. This documentation will include a copy of the tracking system in which each student’s academic adjustments are described, the name and position title of the staff member responsible for ensuring that those academic adjustments were provided as required, and a description of any concerns or delays in the provision of those academic adjustments. G. By 12/15/15, OHSU will submit a draft of the memorandum regarding retaliation described in III.D, above, to OCR for review and approval. H. Within 20 school days of OCR’s approval of the written guidancememorandum, OHSU will submit a report to OCR that reflects the University specific actions taken to comply with the notice requirement of section III.D, above. The report will include the name and title of each person that received the specified notice. I.
