REPORTING PROVISIONS. A. By January 6, 2017, the district will provide to OCR its proposed revisions to the notice of nondiscrimination pursuant to Section III.A.1 of this agreement, and the district’s grievance procedures pursuant to Section III.B.1 of this agreement. Within 30 days of receiving OCR’s request for revision(s) to the notice of nondiscrimination and/or the district’s grievance procedures, if any, the district will incorporate OCR’s revisions and submit revised copies for OCR’s final approval. B. Within 30 days of receiving OCR’s final approval of the revised notice of nondiscrimination and grievance procedures, the district will provide to OCR a report evidencing compliance with Sections III.A.2 and III.B.2 of this agreement. The report will, at a minimum, include a copy of the notification(s) to students, parents, and employees of the revised notice and procedures; a link to the location of the revised notice and procedures on the district’s website; a narrative description of where the summary of the revised notice and procedures were posted throughout the district’s schools; and a copy of any electronic and printed publications containing the revisions. By January 6, 2017, the district will notify OCR of the name, title, and qualifications of the individual(s) designated as its Title IX coordinator(s) pursuant to Section III.C.1 of this agreement. Within 30 days, OCR will notify the district of any areas of concern under Title IX presented by the individual(s) selected as the Title IX coordinator(s) (such as potential conflicts of interest) or with the responsibilities of the Title IX coordinator(s) as described by the district. The district will address any areas of concern presented and notify OCR of its action within 30 days. D. By January 20, 2017 the district will provide OCR a copy of its training program for the Title IX coordinator(s) pursuant to Section III.C.2 of the agreement. 3. By 30 days following the meeting described above in section III, the district will submit to OCR a copy report establishing that the district convened a meeting/meetings of its training materials for XXXXXXX staff pursuant the student’s IEP team and determined the nature and extent of compensatory education, if any, to Section III.D.1 be provided to the student, as required by section III.A and III.B. or if the parent(s) have not agreed to a meeting, that the district offered, the district will document same.
4. If the parent(s) agree to the meeting(s), t The report will include a description of the steps the district took to determine the nature and extent of the compensatory education, if any, and the method to be used to provide any compensatory education to the student. If OCR requires changes to the report, the district will re-submit the report within 30 calendar days of receiving notice of the required changes. The district and OCR will continue this agreementprocess until OCR approves the report.
5. Within 30 days of receiving OCR’s request for revision(sapproval of the report, the district will submit a report to OCR demonstrating that: 1) to the training programdistrict offered the compensatory education contained in the report, if any, to the parents of the student; and 2) the district provided the parents no less than 14 days to respond to the offer. The report will also contain the parents’ response, if any. If the parents did not accept the offer of compensatory education, or if the parents did not respond to the offer within the designated time frame, the district will incorporate OCR’s revisions and submit a revised copy for OCR’s final approvalnot be responsible to provide the compensatory education contained in the approved report.
G. Within 30 6. If the parents accept the offer of compensatory education contained in the approved report, within 120 days of receiving OCR’s final approval of the training program described in Section III.D.1 of this agreement, or no later than March 6, 2017parents’ acceptance, the district will provide documentation to OCR a report evidencing establishing that it delivered the training is provided compensatory education to the designated XXXXXXX employees pursuant to Section III.D.2 of this agreementstudent consistent with the approved report.
H. By March 22, 2017, the district will provide age-appropriate instruction to all XXXXXXXXXXX students pursuant to Section III.E of this agreement.
I. Specifically, the report will include (1) the names or identifying numbers of sending all students at Lincoln Elementary School and Xxxxxx Elementary School whose parent and/or district staff requested that an evaluation of the letter student be conducted to determine whether the student had a disability that required special education or related aids and services; (2) the date on which the request was received; (3) who made the request; (4) the response to the request and the date of the response; (5) if the student was not referred for an evaluation, what information was considered in making this determination and whether the student’s parents parent received notice of procedural safeguards; (6) if the student was referred for an evaluation, the date on which the evaluation of each student commenced and the date on which the evaluation of the student was completed; and (7) the determination regarding whether the student was identified as needing special education or related aids and services.
E. By June 30, 2014, the district will provide OCR with a report reflecting the actions taken pursuant to Sectionsection III.C.2, above, for any students for whom a parent and/or district staff, during the 2012-2013 school year, requested an evaluation but there was no decision or response regarding the request to evaluate. The report will include a description of any compensatory services for any student found to be adversely affected by any undue delay in the district’s referral or evaluation practices. The district will not be required to take action regarding those identified students who no longer attend the district.
F. By September 15, 2014, the district will provide OCR with a report reflecting the actions taken pursuant to section III.C.3 regarding the 2013-2014 school year. This report will include the same specific reporting requirements outlined at section IV.D, above. The district will not be required to take action regarding those identified students who no longer attend the district.
G. By November 15, 2014, the district will provide OCR with a report reflecting the actions taken pursuant to section III.D. This report will include (1) the name or identifying number of each student whose IEP was reviewed;
