Reporting Requirement 4. By August 1, 2017, the District will provide to OCR for review and approval a draft training to OCR. This draft training will include, at a minimum: the name, credentials, and contact information of the trainer; a detailed outline of the content of the training; copies of any proposed materials to be used in the training, such as PowerPoint slides, handouts, etc.

Reporting Requirement 4. If Dallas ISD determines that the male and female athletes at WTWHS are not receiving comparable benefits and opportunities with respect to locker rooms, practice fields and facilities, and competitive fields and facilities, then, within 120 calendar days following recipient of OCR approval of the Facilities Plan, and by the same date thereafter on a yearly basis, the Dallas ISD will submit a report on its progress toward implementing the Facilities Plan. The Dallas ISD will provide OCR with a final report no later than 2 years from the date of OCR’s approval of the Facilities Plan, containing information that demonstrates that it is in compliance with the requirements of Title IX with regard to the equivalent provision of athletic benefits and services. Each report will include appropriate evidence to demonstrate that the plan is being implemented.

  • Reporting Requirement (1) In the event the Contractor identifies covered telecommunications equipment or services used as a substantial or essential component of any system, or as critical technology as part of any system, during contract performance, or the Contractor is notified of such by a subcontractor at any tier or by any other source, the Contractor shall report the information in paragraph (d)(2) of this clause to the Contracting Officer, unless elsewhere in this contract are established procedures for reporting the information; in the case of the Department of Defense, the Contractor shall report to the website at xxxxx://xxxxxx.xxx.xxx. In addition, the Contractor shall describe the efforts it undertook to prevent use or submission of covered telecommunications equipment or services, and any additional efforts that will be incorporated to prevent future use or submission of covered telecommunications equipment or services.
