Required Format. In making use of the Trustmark, Winegrower shall:
1. Always use the Trustmark in the format and to the specifications set forth in Exhibit 3, which Certifier may modify from time to time;
2. Always display the trademark notice™ at the upper right-hand corner of the Trustmark in all signage, labels, packaging, containers, and advertising and promotional materials;
3. If Winegrower is a member of the Foundation, clearly and conspicuously disclose that Winegrower is a member of the Foundation wherever the Trustmark is used;
4. Display any other notices and legends with respect to the Trustmark as may be reasonably requested by the Certifier from time to time.

1. Proposal Content and Organization Direct reference to pre-prepared or promotional material may be used if referenced and clearly marked. Promotional material must be minimal. The proposal must be organized and indexed in the following format and must contain, at a minimum, all listed items in the sequence indicated.

2.11.1 RFP Forms The Lead State’s Request for Proposal forms, reference Attachment C, completed and signed.
2.11.2 Executive Summary The one or two page executive summary is to briefly describe the Offeror's Proposal. This summary should highlight the major features of the Proposal. It must indicate any requirements that cannot be met by the Offeror. The reader should be able to determine the essence of the Proposal by reading the executive summary.
Required Format. All appeals must be in writing and include any supporting documentation that the Respondent (or Reporter in 02. Hazing or 04. Personal Abuse related cases) wishes to be considered. Deference is given to the original Hearing Administrator or CSB’s findings of fact and decision of responsibility and/or any sanctions, therefore the burden of proof is on the Respondent (or Reporter in 02. Hazing or 04. Personal abuse related cases) filing an appeal to sufficiently demonstrate cause to alter procedures, the original decision, or any sanctions. An appeal will generally be limited to an administrative review of the verbatim record of the hearing and supporting documents for one or more of the purposes below, provided however the administrator may request additional information or clarification from the College investigator, Conference administrator, the person filing the report on behalf of the College, the Hearing Administrator or the CSB, Respondent, Reporter, and/or witnesses for purposes of this review.

Required Format. Acceptable format for submitting information to NTC as prescribed by NTC and on transaction forms prescribed by NTC. Signature Either the Original Signature or an Original Signature that has been replicated by photocopy, electronic means, or fax. Successor The trustee or custodian appointed by the Sponsor who succeeds NTC. Written Instruction(s) Any notices, instructions or other instruments required to be in writing (with Signature or Original Signature, where so indicated) from NTC, Sponsor, or its designee. Written Instructions may take the form of a letter, electronic communication through an on-line communication system mutually agreeable to the parties; or a facsimile transmission.
Required Format. Proposals must be submitted as outlined below. The originals shall be clearly marked as such on the front of the binder. (If your proposal has confidential information write/print: Original Unredacted; Original Redacted.) Each portion of the proposal (technical/cost) must be submitted in separate binders and must be prominently displayed on the front cover. Envelopes, packages or boxes containing the original and the copies must be clearly labeled and submitted in a sealed envelope, package, or box bearing the following information: Name of Offeror RFP Number Closing Date and Time Offerors must deliver:
Required Format. Vendor is responsible for ensuring that PCA has the appropriate legal company name, a DBA (“Doing Business As”) if one is used, authorized representatives, and contact information on file for the purpose of receiving notices, changes, addenda or other critical information. Vendors shall submit responses with the RFP # and Title located in the heading on the left margin on all uploaded pages. Company Name shall be in the heading on the right margin. Uploaded documents must be in .pdf format. The entire solicitation has been provided in Bonfire for those Vendors who wish to be able to review the whole solicitation at one time without downloading individual Tabs. However, responses to the solicitation must be uploaded in Submissions separately for Evaluation Committee review. The following Six (6) tabs are required to be submitted in Bonfire in Submissions. Tab 1-Notice of Intent Tab 2-Instructions, Definitions and Evaluation. Tab 3-Contract Terms and Conditions. This action confirms that Vendor has read Tabs 1, 2, and 3. Tab 4-Signature Forms and Vendor Questionnaire/Company Profile.
Required Format. To ensure consistency, Proposer must submit an electronic version and five (5) hard copies of the Proposal clearly labeled “Proposal for Marina Redevelopment and Operation”. The Proposal should be individually tabbed for each of the items or parts listed below and include page numbers. The electronic version of the original proposal shall be in PDF or Microsoft Office format. Both the hard copy(s) and electronic copy(s) shall clearly identify the Proposer and the be labeled “Proposal for Marina Redevelopment and Operation”. In this section, Proposer must describe the proposed composition, organization and management of the redevelopment and operations team including: • Identifying the lead entity (including key individuals and their roles/responsibilities) who will execute the lease agreement with the City. • Identifying other entities (including key individuals) who are expected to be a part of the redevelopment team along with their responsibilities (recognizing that the redevelopment team may evolve as needed to meet applicable requirements related to the selection of contractors and the purchase of professional services, see SECTION 14 for additional information);
