RESPONSIBILITY OF EMPLOYEES. It shall be the responsibility of all nurses to notify the appropriate Director(s) or designate(s) as the case may be, in writing, in duplicate, within three (3) days of any change in her/his address or telephone number. It is understood that any notice sent to a Nurse's last address and name recorded with the Employer shall be deemed to have been received by the Nurse.
