Right to Continue Certain Insurance Benefits. The Individual shall have the right to continue after the Separation Date his group health, dental, and vision insurance benefits, if any, for himself and his dependents, at his own expense (except as provided in paragraph 5 below) in accordance with the Consolidated Omnibus Budget Reconciliation Act of 1985 (“COBRA”). The Individual should complete an insurance continuation election form, which will be furnished to him under separate cover, and timely return it if he wishes to apply to continue his insurance coverage under COBRA.

In accordance with the Consolidated Omnibus Budget Reconciliation Act (“COBRA”), the right to continue after the Company’s first regularly scheduled payday following the Separation Date his group health, dental, and vision insurance benefits, if any, for himself and his dependents, at his own expense. The Individual should complete an insurance continuation election form, which will be furnished to him under separate cover, and timely return it if he wishes to continue his insurance coverage under COBRA.
Right to Continue Certain Insurance Benefits. In accordance with the Consolidated Omnibus Budget Reconciliation Act (“COBRA”), Xx. Xxxxxxx shall have the right to continue after the Effective Date his group health, dental, and vision insurance benefits, if any, for himself and his dependents, at his own expense during the applicable COBRA continuation period.

Executive shall have the right to continue after the Date of Termination his group health, dental, and vision insurance benefits, if any, for himself and his dependents, at his own expense (except as provided below in Section 5 of this Agreement) in accordance with the Consolidated Omnibus Budget Reconciliation Act (“COBRA”). Executive should complete an insurance continuation election form, which will be furnished to him under separate cover, and timely return it if he wishes to apply to continue his insurance coverage under COBRA.
