RISKS AND HAZARDS. 1. The (Unit/Subdivision) is aware of the risks and hazards inherent in entering upon and using DOE NTC facilities, and hereby elects voluntarily to enter upon said premises with full knowledge of those risks and hazards and acknowledges that claims for injury, loss of property, and personal injury caused by the negligent acts, wrongful acts, or omissions by any Federal employee acting within the scope of employment will be resolved pursuant to the Federal Tort Claims Act (28 U.S.C. §§ 1346(b)(1)).

RISKS AND HAZARDS. As part of the occupancy and use of the license area, the Licensee has a continuing responsibility to identify and report all known or observed hazardous conditions on or affecting NFS lands that would affect the improvements, resources, or pose a risk of injury to individuals. Licensee will xxxxx those conditions, except those caused by third parties not related to the occupancy and use authorized by the License. Any non-emergency actions to xxxxx such hazards on NFS lands shall be performed after consultation with the Forest Service. In emergency situations, the Licensee shall notify the Forest Service of its actions as soon as possible, but not more than 48 hours, after such actions have been taken. Whether or not the Forest Service is notified or provides consultation, the Licensee shall remain solely responsible for all Licensee abatement measures performed. Other hazards should be reported to the appropriate agency as soon as possible.
RISKS AND HAZARDS. Craggy Range will make the Contractor aware of existing risks and hazards on site within the area of control or operation of the job. The Contractor will identify risks that it brings onto site and assess and control risks associated with activities undertaken as part of the contract health and safety plan. The Contractor must conduct regular planned inspections to identify risks. All newly identified risks introduced by the Contractors’ activities, impacting on other activities, must be immediately reported to the designated Craggy Range Job Manager. During work which could create a risk, reasonably practicable steps necessary to control the hazard and risks must be taken by the Contractor.
RISKS AND HAZARDS. Participation in the program may expose my child to many risks and hazards, some of which are foreseeable, some of which are not, some of which are inherent in the nature of the program, and some of which may rise from human error or negligence. COVID-19 RISK: Participation in the program may also expose my child to the infectious agent SARS-COV-2 causing the potentially fatal disease COVID-19. The Gabriola Recreation Society has developed a number of COVID-19 related policies and procedures which are available for my review at GRS Program Guidelines . The risk remains, however, that despite the best efforts of the Gabriola Recreation Society, its directors, officers, employees, instructors, volunteers or members. GRS and my cooperation and that of my child, a COVID-19 outbreak could occur. In consideration of GRS allowing my child to participate in the program, I agree: ● To assume all risks arising out of, or associated with, or related to my child's participation in the program even though such risks may have been caused by the negligence of GRS; ● To be solely responsible for any injury, loss or damage which my child might sustain while participating in the program even though such injury, loss or damage may have been caused by the negligence of GRS. I have carefully read and fully understand the terms of this ASSUMPTION OF RISK and PERMISSION AGREEMENT and by signing it voluntarily, I am agreeing to its terms. The ASSUMPTION OF RISK and PERMISSION AGREEMENT is a legal document and is binding on me as well as upon my heirs, executors, representatives and assigns.   • Risks This notification is missioned to reveal to the Customer the information regarding risks connected with conducting trading operations on the financial markets and to warn the Customer about possibility of financial losses related to these risks.   • Additional Insured Status and Scope of Coverage The County of Los Angeles, its Special Districts, Elected Officials, Officers, Agents, Employees and Volunteers (collectively County and its Agents) shall be provided additional insured status under Contractor’s General Liability policy with respect to liability arising out of Contractor’s ongoing and completed operations performed on behalf of the County. County and its Agents additional insured status shall apply with respect to liability and defense of suits arising out of the Contractor’s acts or omissions, whether such liability is attributable to the Contractor or to the County. The full policy limits and scope of protection also shall apply to the County and its Agents as an additional insured, even if they exceed the County’s minimum Required Insurance specifications herein. Use of an automatic additional insured endorsement form is acceptable providing it satisfies the Required Insurance provisions herein.

  • Insured The contractor/renter must be specifically listed as the Insured. OR
