Risk of Injury. Risk of serious bodily harm, injury, paralysis, or death, as well as damage to my equipment and personal property, may occur with respect to my participation in the ISA Event, including, but not limited to, activities related to climbing, aerial lifts, the use of equipment and facilities, officiating, and proctoring.

Risk of Injury. On behalf of my Child, I acknowledge and understand that my Child may be injured by participating in the Activity. Such injuries may include, without limitation, physical or psychological injury, pain, suffering, illness, disfigurement, temporary or permanent disability (including paralysis), economic or emotional loss and death. I understand that these injuries or outcomes may arise from my Child’s negligence or that of others. On behalf of my Child, I understand that any of the aforementioned injuries can occur anywhere in the facility where the Activity takes place such as bathrooms, locker rooms, stairwells and offices of such facilities. Nonetheless, on behalf of my Child and myself, I assume all related risks, both known or unknown to me. RELEASE. We are able to offer some support through practitioners such as physiotherapy or voice specialists, but sustained or lengthy treatment cannot be guaranteed.

I understand all of the rules and regulations. I understand that there are risks and dangers inherent in using the Weight Room facilities. I am fully aware of and understand all such risks. I understand that any failure to follow posted guidelines on facilities use, may place my/my son's health and well being at risk, and could result in serious injury to myself or my son, such as, but not limited to back injury, injury to limbs and body, heart attack or even death. I understand and agree that my/my son's failure to follow any facility rules or regulations may result in the loss of facility privileges.
