SCHOOL-TO-WORK PROGRAM REQUIREMENTS – PROJECT SEARCH. S2W Services using the Project SEARCH model shall comply with the program requirements in the S2W Services general model, in addition to the requirements of the Project SEARCH model. Meet eligibility requirements in Exhibit III, Section III. A. All students must be identified through the Seattle School District accepted by <insert project search title> Project SEARCH, and approved by the County. The Contractor shall: Work with the District to identify and assign employment consultant(s) based on a 40-hour week 12-month position(s) dedicated to Project SEARCH model services, as specified on the S2W Payment Schedule located at xxxx://xxxxxxxxxx.xxx/DCHS/contracts and as negotiated with the County. The employment consultant(s) will meet with the District to identify and coordinate shared responsibility with respect to providing CRP Community Based Assessment, Job Placement, and Intensive Training services to students. These activities shall be documented through staff logs, case notes, or other similar documents. Notify King County, in writing, of staffing changes; such notification shall occur within 1 business day. Serve all District referred eligible students; the Contractor shall provide an employment assessment and place in permanent paid employment.

  • Return to Work Program When feasible, the Employer will provide alternative work opportunities to employees injured on the job. The Employer shall work closely with the employee and his/her physician to determine if and when the employee can return to modified duty, and what assignments and/or activity level restrictions must be adhered to.

  • Modified Work/Return to Work Programs The Hospital and the Union recognize the purpose of modified work/return to work programs, is to provide fair and consistent practices for accommodating nurses who have been ill, injured or permanently disabled, to enable their early and safe return to work. The parties undertake to provide safe and meaningful employment for both permanently or temporarily disabled nurses based on the following principles:

  • Return to Work Programs (a) The parties recognize that prevention of injuries and rehabilitation of injured employees are equally important goals. The parties further recognize that return to work programs are part of a continuum of injury prevention and rehabilitation.

  • WORK PROGRAMME 1. The Parties shall enter into discussions on:

  • Joint Network Implementation and Grooming Process Upon request of either Party, the Parties shall jointly develop an implementation and grooming process (the “Joint Grooming Process” or “Joint Process”) which may define and detail, inter alia:

  • Schematic Design Phase Services § 3.2.1 The Architect shall review the program and other information furnished by the Owner, and shall review laws, codes, and regulations applicable to the Architect’s services.

  • Authorized User’s Statement of Work A competitive Mini-Bid is required for every transaction under this Centralized Contract. An Authorized User must prepare a detailed Statement of Work using Appendix F, Attachment 1, Mini-Bid Template. The Authorized User must distribute the Mini-Bid to all qualified Vendors per Lot(s) (unless a Vendor has removed itself from consideration via the Appendix F, Attachment 5, Mini-Bid Participation Interest Template). Contact information, organized by Lot, will be available on the OGS website for this Contract. An Authorized User shall conduct its Mini-Bid in accordance with the requirements set forth in Appendix F, Attachment 2, How to Use this Contract. The following terms and conditions shall apply to each Mini-Bid issued by an Authorized User:  An Authorized User may require the execution of unique forms, such as Confidentiality Non- Disclosure agreements; and  An Authorized User is required to make tentative award and non-award notifications to each Contractor who submitted a response to the Mini-Bid.   • Modified Work Program The Employer shall provide modified work for any employee injured on the job as per the requirements of the Workers’ Safety and Insurance Board. The employee must co-operate in developing and participating in, a modified work program suited to his capabilities, and with the approval of his physician and/or the WSIB. Both the Employer and the employee shall work together to return the employee to good health and his regular duties.

  • Detailed Description of Services / Statement of Work Describe fully the services that Contractor will provide, or add and attach Exhibit B to this Agreement.

  • Curriculum Work Service on a District curriculum committee shall be required for grade level or department chairpersons. All other teachers working on such committee(s) shall be designated (in writing) by the Curriculum Director and shall be paid, at the hourly rate contained in Schedule B, for all documented hours of committee service (up to such limits as may be imposed by the District) occurring when school is not in session (e.g. during the summer), during duty-free lunch periods or preparation periods, and before/after the teacher’s regularly scheduled work day. Curriculum committee work shall occur at such times as are determined or approved by the Curriculum Director.
