Common use of Single Coverage or HSA Plans Clause in Contracts

Single Coverage or HSA Plans. Effective July 1, 2018 Single Employee 1+ Family High/ Value $574.24 HSA Plan $436.03 $854.96 $1,367.94 District contributions to the HSA trust account will be made each month, to the HSA trust account recommended by the School District Insurance Advisory Committee. Contributions for July will be made at the same time as the August contribution. In the event of hardship, the parties agree to meet and confer to discuss alternatives to the contribution timelines. ● Single HSA Coverage: $200 per month ● Employee+1 Coverage: $400 per month ● Family Coverage: $400 per month The School District will pay all administrative fees associated with the plan.
