Status Change Notification. It is the employee’s obligation to notify the Employer in writing of any change in the employee’s name address, telephone number, tax status, marital status (for benefit and tax withholding purposes only), addresses and telephone numbers of dependents and spouse or former spouse (for insurance purposes only), beneficiary designations for insurance, disability, and pension plans, person(s) to be notified in case of emergency, and any factors that affect the employee’s ability to carry out his/her duties.
