Employment staff complete the NYS Assessment Form (LDSS-4980), and the NYS Employment Plan (LDSS-4978). Group meetings may be scheduled in the future, to also include completing the orientation, basic skills testing and meeting with an Employment Specialist to complete the NYS Assessment Form and Employment Plan on that date. Employment staff review the NYS Assessment form with each individual, and use this information to determine work readiness. Non-exempt individuals determined work ready are assigned to job search and job readiness workshops as appropriate. Individuals who are non-exempt, but not work ready are assigned to workshops and/or referred to other services to address their needs. At the point the individual is determined work ready, s/he is assigned to job search and/or appropriate workshop activities. Further work activity requirements, that may be assigned at a later date, are also discussed with the individual, such as educational training, voc ed, job skills training and work experience. Other available resources are also discussed with the individuals as appropriate. Those completing the Employment Assessment with Xxxxxx Staff are also made aware of the resources available through the One Stop youth programs to assist them to meet their employment goals. At this time, some of the CM's are required to be assessed completing the local equivalent tool with direct data entry the client and some are entering directly into WTWCMS during the client's assessment appointmentWTWCMS. The assessment process is started while consists of reviewing the clients English fluency, education levels, basic skills, work history, training interests, family circumstances (any special needs), medical/mental health status, and supportive services that may be needed, such as child care. The CM also completes the Employment Plan (Attachment D) during this appointment. If a client indicates they are work limited or unable to work, the CM will provide the appropriate document for the client to have completed by their physician and returned within the required timeframe. b. Described below is the district procedure for the completion of an employment assessment: Applicants of cash assistance are engaged by the WD unit the day they apply for benefits. All TA cash applicants/recipients are , including 16 and 17 year olds not attending school, must attend a group activity called Exploring Employment. This activity begins their assessment by listening to the employment requirements and activities they may be required to be assessed or voluntarily participate in and also the support services they may receive while participating. During Exploring Employment, the applicants will complete the JOBS Program Participant Intake Form and also complete the Test of Adult Basic Education. A review of basic job search techniques is also covered. For those individuals that are non-exempt at the time, they are scheduled for four (4) consecutive weeks of Supervised Job Search. Following the completion of SJS or once the case is opened the non-exempt customers are scheduled for an intake with direct data a Workforce Development Counselor for further assessment and development of an employment plan and entry into WTWCMS during the client's assessment appointmentWTWCMS. The assessment process is started while the client is still an applicant and the E&T Specialists Exempts are excused from Job Search but will address barriers, strengths, weaknesses, as well as short term and long term goals.
