TENANT MUST BE A CAREGIVER OR LOWER. INCOME HOUSEHOLD To rent the ADU, the Tenant must be a Caregiver. As of executing the Lease and this Addendum, the Tenant satisfies the category initialed below: Caregiver: The Tenant is a caregiver providing regular care to an owner or occupant of the primary unit in need of that care. Initial if applicable: Initials of Tenant Initials if Landlord Lower Income Household: The Tenant’s household: (i) has gross annual income not exceeding eighty percent (80%) of the San Diego County median, adjusted by household size; and (ii) after the first lease year, has gross annual income not exceeding one hundred percent (100%) of the San Diego County median, adjusted by household size. Initial if applicable: Initials of Tenant Initials if Landlord Landlord and Tenant have confirmed that Tenant is a Caregiver or a Lower-Income Household by completing Occupancy Certification Forms. The Forms with be provided to the City along with this Addendum.

Initial if applicable: Initials of Tenant Initials if Landlord Lower Income Household: The Tenant’s household: (i) has gross annual income not exceeding eighty percent (80%) of the San Diego County median, adjusted by household size; and (ii) after the first lease year, has gross annual income not exceeding one hundred percent (100%) of the San Diego County median, adjusted by household size. Initial if applicable: Initials of Tenant Initials if Landlord Landlord and Xxxxxx have each acknowledged and confirmed under penalty of perjury pursuant to the laws of the State of California that Tenant is a Caregiver or a Lower-Income Household and has truthfully completed the Occupancy Certification Forms. The forms with be provided to the City along with this Addendum and may be subject to verification by the City.

  • Building Safety There shall be a Labor/Management Committee concerning the safety of State Buildings. The Committee shall be made up of one representative from each of the following bargaining units: Representing Labor - Administrative Services; Operations, Maintenance and Support Services; Law Enforcement; Professional and Technical Services; Supervisory Services; Institutional Services; and State Police Unit and an equal number of management representatives selected by the Governor. The Contractor shall ensure the appropriate hot water, heating, and ventilation is provided at the Property during the Program, inclusive in the prices set forth herein.

  • Prohibition Against Selecting and Installing Products Containing Hazardous Materials The Contractor shall not select, install or otherwise incorporate any products or materials containing Hazardous Materials within the boundaries of the Site. Should the Contractor or any Subcontractors have knowledge that, or believe that, an item, component, material, substance, or accessory within a product or assembly selected by the Contractor or any Subcontractor may contain Hazardous Materials it is the Contractor’s responsibility to secure a written certification from the manufacturer of any suspected material which identifies the specific Hazardous Material(s) contained, together with the Material Safety Data Sheets (MSDS) for such materials which shall be submitted to the Owner and Design Professional.

  • Emergency and urgently needed care outside the service area Professional services of a physician, emergency room treatment, and inpatient hospital services are covered at eighty percent (80%) of the first two thousand dollars ($2,000) of the charges incurred per insurance year, and one-hundred percent (100%) thereafter. The maximum eligible out-of-pocket expense per individual per year for this benefit is four hundred dollars ($400). This benefit is not available when the member’s condition permits him or her to receive care within the network of the plan in which the individual is enrolled.

  • Leave for Storm Or Hazardous Conditions It is the responsibility of the Nurse to make every reasonable effort to arrive at their work location as scheduled, however, during storm conditions, when such arrival is impossible, or delayed, all absent time will be deemed to be leave, and the Nurse has the option to:
