Common use of Verification of Illness Clause in Contracts

Verification of Illness. Sick leave absence in excess of 3 consecutive work days must be verified in writing by the employee's doctor. In lieu of such medical verification, the Department Head may accept the employee's signed statement concerning the nature of the illness. Supervisors may require a signed statement from the employee explaining the nature of the employee's illness in cases of sick leave absence of 3 days or less. Where the District has a reasonable basis to believe that an employee's physical condition should preclude returning to work following an absence due to illness or non-industrial injury in excess of 5 consecutive work days, the District may require a physical examination by a physician, who is mutually agreed on by the District and the employee, at District expense. In the event mutual agreement cannot be reached, the parties shall request a reference from the Occupational Health Service.
