XXXX (Supplemental Employment Insurance Benefit) Sample Clauses

XXXX (Supplemental Employment Insurance Benefit). An employee who is a birth mother who has applied for and is in receipt of Employment Insurance maternity benefits is entitled to receive XXXX Plan payments during her maternity leave. The XXXX Plan is intended to supplement the Employment Insurance benefits received by the employee while she is temporarily unable to work as a result of giving birth. Provided she submits satisfactory medical evidence to the Employer, the employee will be entitled to receive XXXX Plan payments for up to eighteen (18) consecutive weeks. While the payments are attributable to the specific period during which the employee is unable to work, she may choose to receive such payments prior to the birth of the child, including during the two (2) week waiting period. The employee will be eligible for XXXX payments for up to eleven (11) additional weeks provided: she continues to receive Employment Insurance benefits, she is unable to work due to a valid health reason related to the birth of the child, and she provides medical evidence satisfactory to the Employer. The XXXX Plan payments will be based on the difference between the Employment Insurance benefits received plus any other earnings received by the employee and ninety- five percent (95%) of her base weekly earnings.

XXXX (Supplemental Employment Insurance Benefit). Birth mothers who are entitled to maternity leave and who will apply for and will be in receipt of Employment Insurance benefits are eligible to receive one Supplementary Employment Insurance Benefits (XXXX) payment equal to one hundred percent (100%) of the employee’s gross weekly earnings during the two (2) week Employment Insurance waiting period.

  • Retirement Insurance A teacher retiring from the District and under the provisions of Teachers’ Retirement Association (TRA) is eligible to continue to participate in group insurance programs (health insurance, dental insurance, life insurance, supplemental life insurance) as permitted under the insurance policy provisions provided the teacher pay the entire premium for such group insurance programs commencing with the beginning of the retirement (see District Website, Human Resources for specific coverage available). The teacher shall be responsible for paying the monthly premium amounts in advance and on such dates as determined by the District/third party administrator. The right to continue participation in such group insurance programs will discontinue upon the failure of the teacher to pay the premiums to the District/third party administrator, or the expiration of insurance availability under the insurance policy provisions. Since long-term disability insurance coverage replaces salary, and a retiree receives no salary, long-term disability insurance coverage is not available.

  • Life Insurance Benefits A. During the life of this Agreement, the basic life insurance benefit made available to Faculty members shall be calculated as 3 times base annual earnings, rounded to the next highest $1,000, but not more than $225,000. A separate additional benefit up to the amount of the life insurance will be paid for accidental death and dismemberment, or loss of sight. The amount of Life and Accidental Death and Dismemberment/Loss of Sight benefits will be reduced to 65% at age 65, and further reduced (from the original insurance amount) as follows: to 50% at age 70, and 35% at age 75. Basic life insurance and AD&D benefits will be provided with no employee contributions.

  • State Employee Group Insurance Program (SEGIP) During the life of this Agreement, the Employer agrees to offer a Group Insurance Program that includes health, dental, life, and disability coverages equivalent to existing coverages, subject to the provisions of this Article. All insurance eligible employees will be provided with a Summary Plan Description (SPD) called “Your Employee Benefits”. Such SPD shall be provided no less than biennially and prior to the beginning of the insurance year. New insurance eligible employees shall receive a SPD within thirty (30) days of their date of eligibility.
