Community Health Improvement Plan definition

Community Health Improvement Plan and “Community Improvement Plan” and “CHP” each means a long-term, systematic effort to address public health problems on the basis of the results of Community Health Assessment activities and the Community health improvement process. This plan is used by health and other governmental, education, and human service agencies, in collaboration with Community partners, to set priorities and coordinate and target resources. A Community Health Improvement Plan is critical for developing policies and identifying actions to target efforts that promote health and defines the vision for the health of the Community through a collaborative process that addresses the gamut of strengths, weaknesses, challenges, and opportunities that exist in the Community to improve the health status of that Community.
Community Health Improvement Plan means the written plan developed by a local health department with the involvement of key policy makers and the general public to imple- ment the services and functions specified under s. 250.03 (1) (L), Stats., pursuant to s. 251.05 (3) (c), Stats., and the requirements of this chapter.
Community Health Improvement Plan means a formal written plan which includes the roles and responsibilities of all participants as well as a mechanism for accountability for agreed upon goals, objectives and services. The plan is developed through a series of timely and meaningful action steps that define and direct the distribution of essential public health services of community public health providers in a specific countywide or multi-countywide area according to partnerships and processes set forth in this chapter. All plans need to be approved by the Office of Local Health.
