Progress towards achieving the project goals and project requirements specified above will be assessed by specific milestones for each project, measured by particular metrics as presented in the attachment Domain 1 DSRIP Project Requirements Milestones & Metrics.
PPS will submit quarterly reports on progress towards achievement of project requirements as defined in Domain 1 DSRIP Project Requirements Milestones & Metrics.
Domain 1 Project Milestones & Metrics are based largely on investments in technology, provider capacity and training, and human resources that will strengthen the ability of the PPS to serve its target populations and successfully meet DSRIP project goals.
In addition, please be sure to reference the document, Domain 1 DSRIP Project Requirements Milestones and Metrics, which will be used to evaluate whether the PPS has successfully achieved the project requirements.
DPHs must select at least nine projects, of which six are specifically required:• Select at least four projects from Domain 1 (Projects 1.1, 1.2, and 1.3 are required);• Select at least four projects from Domain 2 (Projects 2.1, 2.2, and 2.3 are required); and,• Select at least one project from Domain 3.
The PRIME Projects are organized into three Domains:• Domain 1: Outpatient Delivery System Transformation and Prevention• Domain 2: Targeted High Risk or High Cost Populations• Domain 3: Resource Utilization Efficiency The PRIME program will provide incentive payments to participating entities that commit to implementing 5-year projects within the PRIME domains and as further described in Attachment II -- PRIME Program Funding and Mechanics Protocol.

The responses will be evaluated for completeness and a scoring determination will be made based upon the quality of the response.9.1 is worth 33.33% of the total points available for Section 9.9.2 is worth 33.33% of the total points available for Section 9.9.3 is worth 33.33% of the total points available for Section 9.9.4 is not valued in points but contains information about Domain 1 milestones related to Financial Sustainability which must be read and acknowledged before continuing.
